Medical Questionnaire
Are you currently taking any medications? Please state, e.g. (St John’s Wort, aspirin, warfarin, antibiotics, steroids). *
Are you currently receiving any medical treatment, or do you have any medical conditions? *
Please provide details of any recent operations or injuries (within the last year), including dental work. *
Do you have a history of an anaphylactic reaction? *

Do you have a known hypersensitivity (allergy) to hyaluronic acid or any other injectable dermoaesthetic product? *

Is there any possibility that you are pregnant (including IVF) or are you breastfeeding? *

Do you have any acute or chronic skin disease? *

Do you suffer from herpes simplex (cold sores)? *

Do you have any known allergies? *

Do you suffer from any blood-borne conditions (e.g., HIV, Hepatitis)? *

Do you have any tattoos, raised moles, or skin tags in the treatment area? *

Do you have any history of skin cancer? *

Do you have epilepsy? *
Laser Hair Removal – Consultation Terms & Consent
All treatments are carried out by fully qualified and registered medical or laser practitioners, following strict clinical and manufacturer guidelines.
Appointment times and practitioners are subject to availability and cannot be guaranteed.

Appointments & Attendance
A minimum of 48 hours’ notice is required to cancel or reschedule an appointment. Missed or late appointments may result in loss of the session, with no refund.
Please arrive 5–10 minutes early to complete consultation paperwork and clinical photographs (stored securely and confidentially). Late arrival may result in reduced treatment time or forfeiture of the appointment.

Preparation & Suitability
Treatment areas must be shaved the day before your appointment. Treatment may be refused if the skin is unsuitable due to tanning, irritation, medical conditions, or other contraindications, and the session may be forfeited.
Tattoos, pigmentation, and scars must be fully covered prior to treatment.
Results vary between individuals and cannot be guaranteed. Clients with a history of cancer must be in remission for at least 5 years prior to treatment.

Medical Disclosure
You must disclose all relevant medical history, medications, and skin conditions prior to each treatment. Failure to disclose accurate information may result in adverse effects, for which the clinic cannot be held liable.

Courses & Missed Sessions
Laser hair removal courses must be paid in full in advance, or 50% prior to the first treatment and the remaining 50% before the fourth appointment.
Missed sessions without adequate notice may result in forfeiture of the session or, in cases of repeated non-attendance, forfeiture of the course.

Refunds & Liability
Laser hair removal treatments are non-refundable once commenced. No refunds are offered for missed sessions, unsuitable skin at the time of appointment, or change of mind.
If treatment becomes medically unsuitable, a credit or alternative treatment may be offered where possible.

Children in Clinic
Children under 16 years of age are not permitted in the laser treatment room for health and safety reasons.
